How Healthier SG works

新加坡「健康SG」全國基層醫療計劃 — 運作機制與執業指引總覽

Singapore's national primary-care programme pays family doctors to keep enrolled residents healthy — capitation, structured care protocols, subsidised medicines and a national data spine. This hub explains the mechanics and indexes the complete set of official guidelines an enrolled general practitioner must follow.

The resident's journey

居民流程
STEP 1

Invitation

Eligible residents (citizens and permanent residents, aged 40 and above) receive an invitation and enrol with one family doctor of their choice.

STEP 2

First Health Plan

Co-created in person with the enrolled doctor — health goals, screenings and vaccinations due, lifestyle actions. Free to the resident; the clinic claims a fixed fee.

STEP 3

Subsidised care

Chronic medications at polyclinic-comparable prices with subsidies up to 87.5%, free nationally recommended screenings and vaccinations at the enrolled clinic.

STEP 4

Annual check-in

A doctor-led review each calendar year. Chronic enrollees are seen at least twice a year, at least once in person.

STEP 5

Rewards

Residents earn Healthpoints — 3,000 points (S$20) after onboarding, with further points for completing annual check-ins.

How the clinic is paid

診所收費機制
One-off

First-Plan fee + set-up grant

A fixed fee for the first Health Plan discussion (once per enrollee; no consultation charge for that visit), plus a one-off IT enablement grant for a certified clinic management system.

Annual

Annual Service Fee

A capitated fee per enrolled resident, tiered by risk profile, scope of care and progress on preventive and chronic care. Computed from care data submitted through the certified system — the fee schedule itself is not published; Parliament's stated benchmark is the S$100 "Care Plus" fee, "similar or higher".

Quarterly

Chronic Enrolment Grant

A quarterly payment for continuous care of chronic enrollees — conditional on at least two chronic visits a year, one in person, at least three months apart. Roughly S$350 million reached the GP sector in 2025 across these streams.

What participation obliges

參與條件

  • Care Reports submitted per protocol — the asterisked fields drive fee computation

Data & technology

  • Certified Healthier SG-compatible clinic management system, on premises
  • National Electronic Health Record accession, view-access and contribution
  • Data quality criteria, cybersecurity standards, data portability rights
  • Health Information Act duties — mandatory national record contribution from 2027

Network & accountability

  • Membership of a Primary Care Network and partnership with the regional healthcare cluster
  • Participation in core national schemes (chronic disease, subsidies, screening, vaccination)
  • Ministry spot-verification of Health Plan discussions with enrollees
  • A registered Family Physician in every clinic from 1 July 2030

The guideline library

指引文件庫

Every requirement an enrolled clinic must follow, numbered and filed. Official documents and faithful captures of official pages are stored locally for analysis; two restricted documents (the Enrolment Programme Agreement and the Standard Operating Procedures) are represented by notes explaining their status and access path.
